Brief description of the problem

The Pharmacy and Poisons Board had mandated Laboratory & Allied, Ltd to recall Magnesium Sulphate 50% W/V, batch numbers 85055, 86718, 88683, 88688, 88690, 89181, 89182, and 89183, following complaints on mislabelling. The label on the secondary packaging does not match the label on the ampoule. The concentration on the secondary pack is 0.5g/ml, while the concentration on the ampoule is indicated as 0.5mg/ml.

Action for healthcare professionals

Quarantine all remaining stock and stop further distribution, sale, issuing, or use of batches 85055, 86718, 88683, 88688, 88690, 89181, 89182, and 89183 immediately and contact Laboratory and Allied Limited Ltd to arrange return.

Action for patients and caregivers

No further action is required by patients, as this is a Retail/hospital Pharmacy and Wholesaler/Distributor level recall, and the product is administered in a healthcare setting.
